Abstract: The present article describes the process used in the implementation of the "Care Programme Approach" (CPA) in a rehabilitation team based in the community. Information regarding operational policy, procedures, and client groups is provided along with the examination of the match between the levels of disabilities and services provided. The results showed an association between the level of disability, living situation, frequency of contact with 'care coordinator' and 'main carer' with no association between number of days spent at day care or contact with the closest relative. In conclusion, the authors have discussed the implications of the findings. (CIRRIE Abstract)
