Hosting Agreement Form
This form is to clarify that your center/organization/program is willing to host the international rehabilitation expert named below. Visits occur as the result of a request from centers/organizations/programs that have a specific purpose for hosting the visiting expert. The purpose might be lecturing, joint research, technical assistance or other forms of collaboration. The host determines how long the traveler will stay and schedules the activities. The host organization provides lodging, meals and local transportation for the traveler. CIRRIE provides airfare for the traveler.
Please complete this form and return with the application to CIRRIE. If you are a secondary host, please return this form to the primary hosting organization as soon as possible, so that it may be included in the application to CIRRIE. A secondary host is an organization that has also indicated that they would benefit from the traveler's expertise. In these cases, the primary hosting site (the one submitting the application) should contact the secondary hosting organization(s) to confirm the number of days they will host the traveler. Support from the secondary site(s) includes funding the traveler's expenses such as lodging, meals and local transportation.

We agree to host the above named traveler by funding lodging, meals, and local transportation while at our center/organization/program. We understand that travelers are expected to remain in the destination country for a minimum of seven days, during which time they will engage in collaborative activities. Longer assignments are encouraged. There is no maximum length of stay. Travelers participating in conferences are expected to stay for the duration of the conference, as well as engage in collaborative activities before and/or after the conference.
